CONSENT FORM FOR COOKING FEVER YOUTUBE HOST APPLICATION 

Introduction:
Thank you for your interest in becoming the host of our Cooking Fever YouTube channel! To proceed with your application, we require your consent to collect, process, and store your personal data as outlined in this form. Please read the following terms carefully before providing your consent. 

1. Purpose of Data Collection
   We collect personal data to evaluate applications and select a suitable host for our game’s YouTube channel. Your information will help us assess eligibility, communicate with you, and facilitate potential collaboration. 
2. Information Collected
   By completing the application form, you will provide us with the following details: 

• Your name (or alias); 

• Your recorded voice and a recording of your face as part of the video submission; 

• Any other information that may be captured in the background of your recording; 

• Contact email address; 

• Age confirmation (18 or over); 

• Cooking Fever player ID; 

• Social media or YouTube channel (if any); 

• Motivation to be the host of our YouTube channel; 

• Experience in creating gaming content; 

• Additional comments or questions (optional). 

3. Data Usage & Processing
   Your data will be used solely for: 

• Evaluating your application for the YouTube host position; 

• Contacting you regarding your application status; 

• Facilitating potential collaboration and in-game rewards; 

• Internal analysis of gaming content creators; 

• If selected, managing and featuring your content on the official Cooking Fever Game YouTube channel; 

• Editing, promoting, and repurposing your submitted video or future content for YouTube-related activities, including thumbnails, highlight reels, and promotional materials. 

4. Data Storage & Security
   Your personal data will be stored securely and may be held in Google Drive. It will only be accessed by authorized personnel. If you are accepted as the YouTube host, your submitted video may be shared publicly on the official Cooking Fever Game YouTube channel. We will not sell, rent, or share your data with third parties without your explicit consent, except where required by law. 
5. Data Retention
   Your data will be retained for as long as necessary to fulfil the purposes mentioned above. If your application is unsuccessful, your data may be kept for future opportunities for no longer than two years unless you request its deletion. 
6. Your Rights
   You have the right to: 

• Access, update, or correct your personal data; 

• Withdraw your consent at any time (this may affect your application status); 

• Request deletion of your personal data; 

• Lodge a complaint with a data protection authority if you believe your data is being misused.

• You agree to the sharing of your submitted video on the official Cooking Fever Game YouTube channel if you are selected as the host; 

• You consent to the editing, promotion, and repurposing of your submitted video or future content for YouTube-related activities, including thumbnails, highlight reels, and promotional materials; 

• You voluntarily consent to the collection, processing, and storage of your personal data as outlined above; 

• You acknowledge that you can withdraw your consent at any time by contacting us; 

• You confirm that you are 18 years or older.
